To get started, you'll need your full legal name, date of birth, Member ID, the Provider Phone Number listed on your insurance card, and — if you are not the primary insured — the primary insured's information as well.

Click the 'Get Started' button on our insurance page, fill out the form, or email everything directly to info@oloacupuncture.com. Once we have your details, we'll provide a clear breakdown of what your plan covers, including your deductible, co-pays, co-insurance, and any visit limits. We should have your information back to you within one business day — please allow up to one week during January, when insurer call centers experience higher volumes.
