You can schedule insurance appointments online or by contacting us by phone or email during clinic hours. New Patients please schedule a Private Room Foundations in Balance 1 appointment.

If you are a returning patient but it is your first time being seen as an insurance patient, you are billing a new plan, or it has been over 6 months since your last appointment, please schedule a Private Room Consultation and Treatment appointment.

All others can schedule a Private Room Return Visit appointment.

We recommend verifying your benefits before your first appointment so there are no surprises. Submit your information on our insurance page by clicking the 'Get Started' button and we'll generally get back to you within one business day. If you would like to be seen ASAP that is not a problem — you will owe the cash rate in the event you are not covered.
